Find Your Health Team

Rural Frontenac, Lennox & Addington (RFLA) Health Teams include primary care providers (about 25 family doctors and 10 nurse practitioners), other health care providers (nurses, social workers, dietitians, respiratory therapists, kinesiologists, practical assistance and community development workers), and many support staff.

Greater Napanee Health Home – a new model of care
The Mighty Docs Family Health Organization in collaboration with the Kingston Community Health Centres (KCHC), Lennox & Addington County General Hospital and the Town of Greater Napanee launched the Greater Napanee Health Home (GNHH) in April 2024. Health Homes are the new model of care introduced by the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) in which comprehensive ongoing primary care is provided through a team-based model.

The GNHH provides access to team-based primary care at multiple sites in the Greater Napanee area, including for those without a family doctor in the area. Physicians and nurse practitioners are supported by nurses, an Allied Health team and physician specialists – this allows all providers to work to their full scope of practice while addressing the care needs of the population.
