To complement the primary care provided at your family doctor or nurse practitioner office, we have two main sites where many services are offered to people of the Rural Frontenac, Lennox & Addington (RFLA) region:

BY WHO? Services are provided by specialist doctors, nurse practitioners, nurses and allied health providers, including dietitians, social workers, kinesiologists, respiratory therapists, practical assistance workers and more.
HOW? To access the services described below, you may need a referral from your primary care provider (family physician or nurse practitioner); OR you may be able to contact the provider or site directly. See below for more information.
WHERE? Some services are provided in-person, others are provided virtually (by phone or video platform). Service delivery is an evolving situation. It depends on the situation. COVID-19 protocols are in place for in-person sessions.
The Napanee Area Community Health Centre Registered Practical Nurse offers advanced foot care services to the residents of the RFLA region. This service is for foot issues in need of immediate attention. Advanced foot care meets a variety of needs:
- Treatment for prevention care, education and testing for individuals with diabetes or pre-diabetes
- Treatment of hypertrophic and hard to treat nails
- Treatment for those with mobility issues
- Ongoing supportive care by Specialized Nurse to assess and treat in a collaborative health care setting

In collaboration with KFL&A Public Health, Kingston Community Health Centre (KCHC) offers these 3 dental services at two locations, in Kingston and in Napanee at the Napanee Area Community Health Centre (NACHC):
1. Healthy Smiles Ontario (Children): Provides continuous dental care for children who are 17 years and under, and meets the individual/familial income or program requirements.
2. Ontario Seniors Dental Care Program: Provides continuous dental work to seniors who are Ontarians aged 65 and over with a net income of $19,300 or less or a couple’s combined annual income of $32,300
3. Urgent Care Clinic (Adult): Provision of urgent dental care for low-income adults with limited or no access to dental care/benefits.

Services to help manage diabetes are offered at Napanee Area Community Health Centre by two Certified Diabetes Educators – a Registered Nurse and a Registered Dietitian. Our Certified Diabetes Educators offer individual counselling to people with diabetes, pre-diabetes or risk of diabetes living in the RFLA region.
Diabetes topics covered by our diabetes educators include:
- Meal planning
- Nutrition and lifestyle management
- Medication education, including insulin start and adjustment
- Foot screening
- Group education (COVID permitting)
- Grocery store tours (COVID permitting)
- Cooking classes (COVID permitting)

At the Napanee Area Community Health Centre we have harm reduction supplies and naloxone kits for people who need them. We have safer injection kits, safer inhalation kits and safer crystal meth kits.
Supplies and pre-made kits are available to all to pick up from NACHC during regular clinic hours.

Indigenous Community Development Worker
Our Grandmother Kate provides services throughout the Frontenac, Lennox & Addington regions, including culturally relevant community development programs that are open to the public. Many programs are on hold due to the COVID-19 pandemic.
Indigenous Nurse Practitioner
One of our Napanee Area Community Health Centre Nurse Practitioners provides culturally relevant care for Indigenous Peoples who are rostered NACHC clients.

Sometimes a specialist is needed to help with your care. We have a few types of specialists affiliated with RFLA Health.
A referral is needed from you primary care provider to see a medical specialist.
Internist
Internal medicine physicians are specialists who apply scientific knowledge and clinical expertise to the diagnosis, treatment, and compassionate care of adults across the spectrum from health to complex illness.
Pediatrician
Pediatric Medicine is the specialty that focuses on the physical, emotional and social health of neonates, infants, children, adolescents and young adults.
Psychiatrist
Psychiatry is the branch of medicine devoted to the diagnosis, prevention, study, and treatment of mental disorders. Psychiatrists can prescribe medication as part of treatment.

A Nurse Practitioner (NP) is an advanced practice registered nurse, who is trained to assess patient needs, order and interpret diagnostic and laboratory tests, diagnose disease, formulate and prescribe medications and treatment plans.
There are several Nurse Practitioners working in clinics throughout the RFLA region.
Our team of NACHC Nurse Practitioners are primary care providers who offer a wide variety of primary health care services to help you achieve your health goals, including:
- consult and collaborate with physicians and other healthcare professionals to meet the needs of patients,
- play a role in health promotion, disease and injury prevention, chronic disease management, cure, rehabilitation, and
- connect you with community programs and services, including specialist care.

Practical Assistance and Community Outreach Workers help people and their families to meet their practical needs. Focusing on the social determinants of health, these workers can help find resources and solutions that could assist clients with their day-to-day living.
The RFLA Allied Health Team Practical Assistance Worker offers support to clients in numerous ways: navigating through government services and documents, making referrals for agency/community resources and advocate in the areas of housing, income security, food security, literacy, transportation and health care access.

The Napanee Area Community Health Centre (NACHC) also has a Community Outreach Worker who provides one-on-one help and group programming to individuals and families, in many areas of practical assistance, including precarious housing.

Registered Dietitians help people understand the connection between their health and their food choices. They use the most up-to-date science while providing personalized nutrition advice and practical solutions, without fads or gimmicks.
The RFLA Allied Health Team Registered Dietitian offers individual nutrition counselling in-person or virtually (by phone or video call) and for all age groups. Counselling can address all nutrition issues, except diabetes. The RFLA dietitian also runs group programs – learn more.

Registered Kinesiologists help people get moving. They apply the biomechanics of the body and principles of physical activity to improve people’s health and quality of life. They factor in the presence of chronic disease and physical limitations when prescribing exercise.
The RFLA Allied Health Team Registered Kinesiologist offers individual counselling in-person or virtually (by phone or video call). Counselling can address: chronic disease prevention and management, exercise prescription and injury rehabilitation.

Registered Respiratory Therapists work with patients with all kinds of breathing or pulmonary disorders, such as asthma, COPD, lung cancer, pulmonary fibrosis and bronchiectasis. Through respiratory assessment and education they support patients and their families in optimizing their lung health.
The RFLA Allied Health Team Registered Respiratory Therapist (RRT) runs the Lung Health Program with individual services and group workshops in-person or virtually (by phone or video call). Services include:
- Spirometry testing – to measure lung volume and airway function, and other respiratory assessment
- Management of asthma and COPD
- Review inhaler use and need for change
- Education on lung diseases, including breathing exercises and techniques how to improve lung health
- Virtual home-monitoring with the Breathe Program, an L&A Hospital initiative, with RRT follow-up
- CPAP Clinics are run periodically to help learn about optimizing use of the CPAP machine used for treating sleep apnea

Registered Social Workers and Psychotherapists provide mental health counselling to help people get the most out of their relationships, work and community life. These mental health counsellors provide therapeutic services to support patients’ social, emotional, and psychological wellbeing from a trauma informed lens. They can help identify the source of stress or problems, strengthen coping skills and find lasting solutions.
These Mental Health Counsellors support individuals, couples and families by addressing a wide range of interpersonal problems, difficult relationships and life transitions. As needed, they will help link people to community groups and resources. Groups and workshops on topics such as managing emotions, adverse childhood experiences, stress management, emotional eating, wellness, and mental health may be available.

Sexual health services specialize in sexual and reproductive health and can include birth control, pregnancy tests and counselling, sexual health education, sexuality counselling and support, as well as the prevention of, detection, and treatment of sexually transmitted infections (STI).
At NACHC we offer several sexual health services, including:
- Birth control start, Plan B and pregnancy tests (call for an appointment with your Nurse Practitioner)
- Sexually transmitted infection testing can be done on NACHC rostered clients who have symptoms. Call for an appointment with your Nurse Practitioner.

Our Registered Respiratory Therapist and Registered Nurse run the Smoking Treatment for Ontario Patients (STOP) program through the RFLA Allied Health Team and the NACHC. This program will help you quit or reduce your smoking. We provide free nicotine replacement therapy and counselling.
To be eligible you must be 16+ (or have consent from parent/guardian), a current smoker of cigarettes (or have quit in the last 30 days) and have an OHIP card. Potential participants will be screened; then asked to complete the STOP registration form with smoking cessation provider prior to starting the program.

The NACHC Community Development Worker for Youth connects individuals with community partners and resources. The Lennox & Addington Youth Services is based out of the Napanee Area Community Health Centre and offers services and programs to youth aged 10 to 25 and their families.
- Services include one-on-one system navigation, referrals and supports for youth and their families.
- Programs include both virtual and in-person groups that are recreational, psychoeducational and focused on youth engagement. See Youth Hub Programs
