Our Privacy Promise

Our health care teams throughout the Rural Frontenac, Lennox & Addington (RFLA) region are committed to protecting your personal health information and respecting your privacy rights.

Personal health information includes your name, date of birth, address, health history and records of your visits to health care providers and the care that you received during those visits. Your medical record contains this information.

When you are seeing one of our providers, we assume that you consent to our collecting, using and sharing your health information to provide you with health care, unless you tell us otherwise.

We collect your personal health information to have a full picture of your health and to offer you quality care. Your information is entered into our RFLA Health Collective electronic medical record (EMR) system.

We will usually only share your information with those who are providing you with health care, such as our doctors, nurse practitioners, residents and other team members. In order to help coordinate your care with other health care providers, if needed, we may also share your information with Ontario Health to update your provincial electronic health record (EHR).

Sometimes we may collect, use or share your health information without your permission, including:
• to get payment for the services we provide to you
• to contact a relative or friend if you are incapacitated
• to report certain diseases to public health authorities
• when necessary to prevent serious harm to you or others
• when we suspect certain types of abuse

Sometimes we get your permission before we collect, use or share your health information for research. However, some research projects do not require your consent under certain conditions. These research projects must be approved by a research ethics board and must have privacy safeguards in place.

We may also use your health information for educational purposes and to plan our services and may share this information with certain organizations for health system planning and management.

We have robust privacy policies and procedures that comply with applicable privacy laws and regulations, including the Personal Health Information Protection Act, 2004, known as PHIPA.

All team members (health care providers, admin staff and providers-in-training) complete RFLA Health Privacy Policy Training and sign our Confidentiality agreement.

We only collect the amount of information needed to perform our duties and only use your personal health information for authorized purposes.

We protect your information from theft, loss, and unauthorized access, copying, change, disclosure and disposal.

We conduct audits to monitor and manage our privacy compliance and conduct investigations when needed.

• ask to look at and get a copy of your health information stored in the electronic medical record
• ask to correct inaccurate or incomplete information in your record
• withdraw consent for some of the above uses by contacting us (subject to legal exceptions)
• know how we use and share your personal health information
• be told if your information is stolen, lost or improperly accessed

To exercise your rights, please speak to your clinic staff or provider.

We are part of a multisite health collective that uses a common electronic medical record (EMR). The key partners in this collective are as follows:

    • Includes the Greater Napanee Health Home sites
  • Rural Kingston Family Health Organization physicians
    • Includes Sydenham, Tamworth, Newburgh & Verona clinics
  • Specialist Physician Partners

We are working toward better-integrated health services for you.  We may use de-identified information from the shared EMR to plan programs, teach, do research, population health evaluation, and other purposes allowed by law.
